INVITATION FOR BIDS

 

 

Sealed bids for the 2012 Asphalt Mill and Overlay Project will be received by the Director of Public Services at the Crestwood Government Center, One Detjen Drive, until Wednesday, February 1, 2012 at 10:00 a.m. CDT when they will be publicly opened and read in the Aldermanic Chambers. 

 

The Scope of Work includes milling of asphalt pavement, placement of asphalt overlay, installation of asphalt paving fabric, base repairs, traffic control, and other incidental items.

 

Bid packages are available from the Department of Public Works at the Government Center free of charge.

 

Not less than the prevailing hourly rate of wages shall be paid to all workmen performing under this contract in this area according to the rates determined by the Department of Labor and Industrial Relations of the State of Missouri.

 

The City of Crestwood is a tax exempt entity.  A project exemption certificate will be supplied to the successful contractor.

 

A bid security in the amount of five percent (5%) of the bid amount must accompany each bid in accordance with the Instructions to Bidders.

The Contractor (and any subcontractor performing any portion of the Work hereunder) shall require all on-site employees to complete a ten (10) hour OSHA construction safety program or similar program as approved by the Public Works Director. Said program must be completed within sixty (60) days of beginning work on the project. Any employee found on the site without documentation of the successful completion of the program shall be given twenty (20) days to produce such documentation before being subject to removal from the site. Failure to comply with this Article will result in the Contractor forfeiting as a penalty the sum of $2500.00, plus one-hundred ($100.00) per day, for each employee employed without the required training. The City may withhold all sums and amounts due and owing as a result of any violation of this Article when making payment(s) to the Contractor.

 

The City of Crestwood shall award any contract based on the proposal that, in its sole discretion, best meets the interests and requirements of the City.  The City of Crestwood reserves the right in its sole discretion to reject any and all proposals, to waive technicalities or deficiencies in any or all the proposals, to negotiate with any or all bidders or others for other or more favorable terms or prices, and/or to award the contract to other than the bidder submitting the lowest cost proposal, with or without negotiation.

Free income tax assistance to senior citizens (ages 60+)

FREE assistance with simple federal and state income tax forms and the Missouri Property Tax Credit Claim (Mo-PTC or “Circuit Breaker”) is available through the St. Louis County Department of Human Services’ County Older Resident Programs (CORP).  Tax assistance is provided by appointment at the following locations:

  • St. Louis Deaf Community Center at 8764 Manchester Road, Suite 201 in Brentwood,
  • Oak Bend Library at 842 South Holmes in Oakland
  • St. Louis Government Center-South, in Mehlville

This service is available to St. Louis County residents who are 60 years of age or older.  Appointments may be scheduled, beginning January 30, 2012, by calling 314-615-4037, Monday through Thursday, 9:00am – 2:00pm.  Persons with hearing impairments may call 314-615-4001.

ELECTION FILING NOTICE

 

Notice is hereby given that, pursuant to the provisions of the Charter and Ordinances of the City of Crestwood, a General Election will be held in the City of Crestwood, St. Louis County, Missouri, on Tuesday, April 3, 2012, for the purpose of the following:

 

       1.   Electing four Aldermen, one from each Ward, for a term of three (3) years,    

which term shall expire in April, 2015.

 

      2.   Electing one Alderman from Ward Two to fill a term which expires in April, 2013.

 

Candidates may file for office and have their names printed on the ballot by filing with the City Clerk, or her designee, a Notice of Candidacy, a Petition containing at least 20 signatures of registered voters and by paying a $5.00 filing fee.  Persons interested in filing for this office may obtain forms and information on this process by contacting the Office of the City Clerk, City of Crestwood Government Center, One Detjen Drive, Crestwood, Missouri  63126 (314) 729-4773.

 

The first date for filing these completed forms with the City Clerk or her designee is Tuesday, December 13, 2011 at 8:00 a.m.  The final date for filing these completed forms with the City Clerk or her designee is Tuesday, January 17, 2012, before 5:00 p.m.   

 

Government Center Offices will be closed for a half day on December 23, 2011, and December 30, 2011.

 

Government Center Offices will be closed all day on December 26, 2011, January 2, 2012, and January 16, 2012.

 

Candidates wishing to file their candidacy forms shall form a line in the lobby outside the door to the General Office at the Crestwood Government Center, One Detjen Drive, Crestwood, Missouri.  Their names will be listed on the ballot in the order of their filing.
